Weyerhaeuser employee dies following accident

COTTAGE GROVE, Oregon - An employee died after becoming trapped in a piece of machinery at a Weyerhaeuser facility on Tuesday.

According to a report from KMTR-TV, the sheriff's office identified the deceased as Justin Simons, age 31.
According to a fundraising website established by friends of the family, Simons is survived by his wife of 9 years and three young children, ages 6, 4 and 3.

South Lane County Fire and Rescue responded to a report of an industrial accident at the Weyerhaeuser mill site in Cottage Grove at 4:20 p.m. on 9 December 2014. Crews arrived to find Simons being resuscitated by an emergency response crew from the mill.

The initial reports prior to arrival of fire crews were that the employee was trapped in a piece of machinery. Simons was free from any machinery when crews arrived.

He died at the scene of his injuries.

The death is being investigated by the Lane County Sheriff's Office, OR-OSHA and Weyerhaeuser.

Simons, a 7-year employee of the mill, reportedly died in the conveyor chain area, a machine that removes sawdust from the facility.

KMTR-TV reported that OR-OSHA investigators visited the mill on Tuesday and again on Wednesday. By law, the agency has six months to complete its investigation.
